CASE MANAGEMENT INTAKE PROCESS:
01. ASSESSMENT
Complete free mental health screening to identify the specific needs and challenges of each with a solution.
02. PLANNING
Based on the assessment, a personalized master treatment plan is developed with input of client, support system, and healthcare providers.
03. MONITORING
Weekly check-ins and assessments are conducted to track milestones and make necessary adjustments to treatment plan.
04. COORDINATION
Client coordinators work with various healthcare professionals and resources to ensure the success of client.
